Case Manager - Secret Garden and Freedom House

Barrier Free Living (BFL) is dedicated to helping New Yorkers with disabilities live independently in the community.  BFL provides a broad range of residential (Freedom House) and non-residential (The Secret Garden) services which provides a variety of services to domestic violence survivors with disabilities.

The Secret Garden offers case management, safety planning, occupational therapy, short- and long-term individual counseling, advocacy within the medical, mental health, child welfare, law enforcement, and criminal justice systems, and referrals to outside services.  These services are provided at Secret Garden’s Manhattan-based office, as well as at New York City-sponsored Family Justice Centers located in the five boroughs.

Freedom House is BFL’s emergency shelter for domestic violence survivors with or without disabilities. Freedom House accepts women, children and men.  Its programs include individual counseling for adults and children, support groups, safety planning, case management, housing assistance, occupational therapy, financial clinic services, onsite childcare, and advocacy within the medical, mental health, child welfare, law enforcement, and criminal justice systems.

This position reports to both locations Freedom House and Secret Garden. This position reports to Freedom House Director of Social Services and Secret Garden Program Director, Assistant Director as needed (SG).

QUALIFICATIONS:

  • BA or equivalent degree
  • Counseling and case management experience in the field of domestic violence and persons living with disabilities preferred
  • Spanish fluency preferred
  • Strong knowledge in navigating organizations, systems and community based resources in New York City
  • Ability to work in crisis management environment
  • Effective communication skills

RESPONSIBILITIES:

  • Coordinate concrete case management services, information, referral and advocacy in collaboration with Social workers and occupational therapists for all survivors with disabilities between The Secret Garden and Freedom House
  • Serve survivors with a physical, psychiatric, cognitive, sensory and/or neurodevelopmental disability
  • Liaison with social services agencies, police precincts and the court systems throughout the five boroughs
  • Conduct telephone screenings of calls from the domestic violence hotline
  • Provide crisis intervention and other services required to assist survivors
  • Complete individualized service plans for survivors related to ongoing recovery, benefits and housing assistance needs, safety planning, and other emerging issues; also ensuring appropriate follow-up activities related to service plan goals and benchmarks
  • Participate in staff/program meetings and trainings, partner program meetings, and conferences
  • Ensure timely and accurate entry of case notes and other related program statistics, as requested, including document and complete the necessary forms at The Secret Garden and Freedom House
  • Conduct outreach and educational presentations to other service providers, policymakers and the general public

Start Date: ASAP
Annual Salary: $43,000.00

Hours per week: 35:00 – Monday through Friday
