Nurse, LPN at Freedom House Program Part Time

Barrier Free Living (BFL) is dedicated to helping New Yorkers with disabilities live independently in the community. Through its various programs, BFL provides a broad range of residential and non-residential services to disabled survivors of domestic violence, transitional shelter to the disabled homeless, and outreach to physically disabled individuals with mental health and/or chemical addiction issues. Services include case management; housing placements; independent living skills training; occupational therapy; short- and long-term individual counseling; advocacy within the medical, mental health, child welfare, law enforcement, and criminal justice systems; and referrals to outside services.

This position reports to Program Director and collaborates with the Director of Social Services and Occupational Therapist.

Responsibilities

  • Performs complete and accurate nursing medical screenings on all new residents
  • Collect and maintain PPD record of residents, as per state regulations
  • Collaboration with interdisciplinary team including but not limited to social workers, case managers, and occupational therapy
  • Initiates appropriate teaching and/or referrals
  • Linking to medical providers
  • Coordinating with resident and medical provider around safety
  • Coordinates services with home care and visiting nurse services
  • Provide outreach and community linkages within the medical community
  • Performs phone screenings for admission into the shelter
  • On-call for medical emergency to advise staff
  • Performs and legibly documents nursing care according to resident needs in a competent and timely manner.
  • Perform emergency interventions, as needed
  • Performs other tasks as appropriate, as per the Program Director

Qualifications

  • Licensed Practical Nurse (LPN) with three year’s experience
  • Preferred experience with people of various disabilities including physical and medical conditions
  • Clearance from the State Central Register of Child Abuse and Maltreatment.
  • Experience with training CPR, First Aid, and AED, preferred
  • Computer literate

Start Date: ASAP

Hour Rate: $33.00

Part Time Hours: 14-21 hours per week
